ESR: The Physics of Falling Coins
ESR stands for Erythrocyte Sedimentation Rate. It measures how fast your red blood cells (RBCs) fall to the bottom of a test tube in one hour
The Mechanism (Zeta Potential)
Normal State: Healthy red blood cells have a negative charge on their surface (due to sialic acid). This creates a repulsive force called Zeta Potential, keeping the cells apart so they float and settle very slowly
Inflammation State: When you are sick, the liver releases "sticky" proteins like Fibrinogen. These proteins neutralize the negative charge
Rouleaux Formation: Without repulsion, RBCs stick together like a stack of coins (Rouleaux). These stacks are heavier than single cells, so gravity pulls them down faster
Result: A high ESR number means your red blood cells are falling fast, indicating significant inflammation
Note: ESR is slow to rise and slow to fall. It is often used to monitor chronic conditions like Rheumatoid Arthritis
CRP: The Chemical First Responder
CRP stands for C-Reactive Protein. Unlike the physical measurement of ESR, this is a chemical quantification of a specific protein produced by the liver
The Mechanism
When the immune system detects a threat (bacteria or tissue damage), it releases a signal called Interleukin-6 (IL-6)
The liver receives this signal and immediately pumps out CRP
